Sarcoïdose

Wat sarcoïdose is:

Een granulomateuze, immuun-gemedieerde ontstekingsziekte. Het kenmerk is de vorming van niet-verkazende granulomen (ophopingen van macrofagen, epithelioïdcellen en T-cellen) in organen zoals longen, lymfeklieren, huid, ogen en soms hart of zenuwstelsel.

Het mechanisme:

Bij sarcoïdose reageert het immuunsysteem overdreven sterk (met name via Th1- en Th17-routes) op een nog onbekend antigeen — mogelijk een omgevingsfactor, micro-organisme, of ander triggerend deeltje bij genetisch gevoelige personen. Er is dus wel sprake van immuundisregulatie, maar niet per se van de klassieke auto-immuunmechanismen waarbij het lichaam antilichamen of T-cellen richt tegen eigen weefsel-specifieke antigenen (zoals bij bijvoorbeeld lupus of reumatoïde artritis)

Waarom de grens met een auto immuunziekte vaag is:

• Er worden bij sarcoïdosepatiënten wel verhoogde auto-antilichamen gevonden bij sommige subgroepen

• Er is overlap in cytokineprofielen met erkende auto-immuunziekten

• Sommige onderzoekers plaatsen het daarom in een tussencategorie: "immuun-gemedieerd" in plaats van strikt auto-immuun of strikt infectieus/reactief

Sarcoïdoseafwijkingen in zenuwen leiden tot stoornissen in de zenuwwerking. 

Dit kan zich uiten in zenuwpijn, gevoelsverlies of tintelingen en krachtverlies in de spieren. 

Men spreekt in deze gevallen van neuropathie.
